Washington Prognosis: House Elects New Speaker; NIH Director Nominee Advances
On Wednesday afternoon, Rep. Mike Johnson (R-LA) was elected as the 56th Speaker of the House, after Rep. Kevin McCarthy (R-CA) was ousted more than three weeks ago.  Johnson was the party's fourth nominee for Speaker, following unsuccessful bids from Reps. Tom Emmer (R-MN), Steve Scalise (R-LA), and Jim Jordan (R-OH).   Rep. Johnson sent a letter to his colleagues prior to his election detailing his proposed timeline to advance fiscal year (FY) 2024 appropriations bills, as well as other must-pass legislation including the Farm Bill and the National Defense Authorization Act (NDAA).  Speaking to the chamber after his election, Johnson emphasized his priorities, including border security, cutting non-defense federal spending, and establishing a bipartisan national debt commission.  With the House now able to resume legislative business, the new Speaker will likely suggest a stopgap measure to extend government funding until either January or April 2024.  This means that, as in the previous few years, negotiations on FY 2024 appropriations may continue well into December.  As part of these negotiations, Speaker Johnson will also have to consider the Biden Administration’s latest request for $56 billion to address a variety of domestic issues including disaster relief, childcare, and national security, in addition to President Biden’s previous request for a $100 billion emergency national security package to provide military and humanitarian assistance for Ukraine and Israel.      

In other Washington news, after weeks of back-and-forth, Dr. Monica Bertagnolli, President Biden’s nominee for the next Director of the National Institutes of Health (NIH), is now headed to a full Senate confirmation after the Senate Health, Education, Labor and Pensions (HELP) Committee voted 15-6 to advance her nomination.  Despite a bipartisan vote, HELP Committee Chairman Bernie Sanders (I-VT) voted against her nomination after expressing disappointment in what he views as Dr. Bertagnolli’s reluctance to make a firm commitment “to take on the greed and power of the drug companies and health care industry”.  Her nomination will now move to the Senate floor for final approval, although the vote has not yet been scheduled. 

 
House Energy and Commerce Committee Holds Hearing on Long-Term Services and Supports
The House Energy and Commerce Subcommittee on Health held a hearing to discuss federal policies focused on long-term services and supports (LTSS), which refer to the services that seniors and people with disabilities often need for their daily care.  Republicans on the Subcommittee criticized two recent proposed rules from the Centers for Medicare and Medicaid Services (CMS) that would set minimum nursing home staffing requirements and require 80 percent of Medicaid payments for specific home- and community-based services (HCBS) to go to the direct care workforce.  Republicans argued that, while well-intentioned, these proposed rules would force additional long-term care facility closures and further limit access to care.  Democrats agreed that future legislation should consider the current workforce shortages across all types of care.  However, they supported the proposed rules and highlighted exceptions for facilities in areas with workforce shortages and argued that the higher wages to caregivers would stabilize the existing workforce.

NIH Announces New Simplified Review Framework
In an effort to reduce bias in the evaluation of research proposals for scientific merit, the National Institutes of Health (NIH) announced the steps that they are taking to simplify the grant review process for research project grants (RPGs).  This change is intended to address feedback NIH has received from the extramural community expressing frustration with both the complexity of the process and the undue influence of reputational bias.  The five criteria that are currently scored will now be reorganized into three factors: 1) importance of research 2) rigor and feasibility; and 3) expertise and resources.  The third factor will not be scored numerically and will only be used to determine if an institution's resources are sufficient to successfully conduct the proposed research.  The NIH originally introduced this framework in 2022 through a Request for Information (RFI), which garnered nearly 800 responses, the majority of which supported the proposed changes.   
 
The new framework will be put into effect for application deadlines of January 25, 2025 and beyond, and NIH plans to develop timelines, training, and policies to support the enactment of the process.  An online briefing will be held on November 3, 2023 from 1:00 to 2:00 pm ET to provide an overview and answer questions regarding the new simplified peer review framework.  Additional resources and guidance for the NIH community can be found on the newly developed Simplified Review Framework webpage

  • USDA Report on Household Food Security in 2022:  The U.S. Department of Agriculture (USDA) Economic Research Service (ERS) released a report analyzing households food security, food expenditures, and the use of federal nutrition assistance programs in 2022.  The report found that 12.8 percent of U.S. households were food insecure in 2022, equivalent to 17 million households.  This is a large increase over the 10.2 percent or 13.5 million households in 2021.  Additionally, 5.1 percent or 6.8 million households reported very low food security, up from the 3.8 percent or 5.1 million households in 2021.
  • Newly FDA-Approved RSV Vaccine for Infants Faces Extreme Supply Issues:  The Pharmaceutical manufacturer Sanofi issued a statement this week concerning elevated demand for the Respiratory Syncytial Virus (RSV) prevention drug Beyfortus, expressing that despite their “aggressive supply plan,” they are facing difficulties in meeting the demand for the drug before the peak of RSV season.  Beyfortus was officially approved by the Food and Drug Administration (FDA) in July 2023.  Sanofi stated that they are “in close collaboration with the Centers for Disease Control and Prevention (CDC) to ensure equitable distribution of available doses through the Vaccines for Children Program (VFC),” and they are working to find solutions to the issue.
